This agreement, made and entered into this date of 00/00/00, between Streamline Productions, hereafter
referred to as the DJ and the customer, referred to as the organization, whereas the DJ and the organization
agree to the following:

Witness, that for and in consideration of the sum of $000 dollars, paid as a non-refundable retainer fee by
the organization to the DJ, receipt of which is hereby acknowledged, the client herewith contracts the
services of the DJ to provide entertainment according to the following terms and conditions listed below and
on the reverse side of this page.

Contract is null and void if not signed and returned by 00/00/00

This contract cannot be canceled by any of the parties hereto. Legitimate reasons for non-performance are
(a) National Emergencies, (b) Acts Of God, and (c) any other reason arising from circumstances beyond the
control of either party. Reason for non-performance must be clearly proven. The organization member or
authorized representative thereof hereby signing this agreement acknowledges his or her authority to do so
and hereby assumes liability jointly and severally with the organization for the implantation of the terms and
conditions stated herein. If either party breaches this agreement, he shall be liable for and pay to the damaged
party the full agreement price, payment of which shall relieve the guilty party from further liability. The
organization further agrees to provide such access no later than one hour prior to performance. Failure to honor
this requirement will in no manner constitute breach of contract or obligate the entertainer(s) to make up any
lost performance time.

The organization hereby assumes responsibility for its guests, customers, and staff, and agrees to maintain
conditions that will not inhibit the performance of the entertainer(s) nor cause loss, injury, or damage to the
entertainer(s) or their equipment. The organization agrees to an ample substitution of entertainer(s), due to
circumstances beyond control, without prior notice to date of performance. The organization and/or its
authorized representative agree to; (a) late charge of fifty dollars ($50.00) for all payments not received
by date noted in terms and conditions: (b) an additional fee of fifty dollars ($50.00) for each check returned
no matter what the cause. The organization and/or its authorized representative agree to pay all processing
and litigation fees to recover payments due for services rendered. The organization understands and agrees
that this provision is a material provision of this agreement that is a part of the consideration involved
in entering into the agreement, and the organization would not have entered into this agreement except for
inclusion of the provision herein. This agreement contains the entire agreements of both parties and is
binding on both parties. Any future modification to this agreement will not be considered binding unless in
writing and signed by both parties.

The organization agrees to have the establishment where the event is to take place provide the DJ with a
standard 8' banquet table with tablecloth that is centered and directly overlooking the dance floor area with appropriate outlets that are no more than 20' from where the DJ is to conduct the performance. The DJ is to be provided an open area that is 10' x 10' (10' front to back and 10' left to right). The organization understands that failure by the establishment where the event is to take place to comply with these requirements may result in non-performance by the DJ at his discretion without having to issue a refund of the retainer fee to the organization.
